Life Plan Community

A Life Plan Community and a Continuing Care Retirement Community (also known as a CCRC) are two names for the same type of community — full-service communities that offer Independent Living and a continuum of higher levels of care. On Long Island, Jefferson’s Ferry was the first Life Plan Community and also the first to offer the Type A Life Care contract, considered the gold standard in senior living residency agreements.

We opened our doors in 2001 and we’re proud of the recognition we’ve earned for overall excellence and an organization-wide focus on wellness. Beyond the awards, our efforts over the past 20 years demonstrate the passion and commitment we bring to helping residents live the life they love.

Jefferson’s Ferry has been consistently recognized by Suffolk County’s largest business group, the Hauppauge Industrial Association, for Outstanding Business Achievement. We have also received “Best of Long Island” awards in several categories (Best Retirement Community, Best Assisted Living, Best Nursing Home) over multiple years, by readers of The Long Island Press.

The Board of Directors of Jefferson’s Ferry is committed to serving older adults by providing an active lifestyle within an all-inclusive Life Care retirement community. The Life Care concept promotes dignity, privacy, individuality and independence for each resident. This is accomplished through a caring environment with concerned staff providing security and peace of mind through a focus on resident service.
